Program Forms and Outreach Materials

You cannot apply for the Safe at Home Program online! You must go to an enrolling agency and complete the application forms in person.

Participant forms must be requested by phone and originals with original signatures must be submitted to Safe at Home by mail to: Safe at Home, P O Box 846, Sacramento, CA 95812. Only certain participant forms may be submitted by fax, please call Safe at Home for more information. All other non-participant forms may be submitted by fax or mail.
